House calls for older adults in Northern New Jersey
For an older adult, a trip to the doctor’s office can take half a day and someone else’s afternoon off. A physician comes to the house instead.
Reaching the doctor
Patients can reach the physician at any hour. Call the office and you’ll be connected. For an emergency, call 911.
What a visit covers
An exam without a clock running, a look at every medication in the house (including the ones in the kitchen drawer), treatment for common problems, and a plan the family can follow.
Alongside your primary care doctor
House calls work alongside your primary care doctor, not instead of them. Our physician keeps your primary care doctor and specialists updated, so care stays coordinated and everyone works from the same plan and the same medication list.
If you or your family member also has a Nexstep aide or nurse, the physician already knows the care plan, because our Medical Director reviews it.
When you need the doctor
- A direct line to the physician, day or night
- Same-day or next-day visits when clinically appropriate
- One physician who knows the full history, medications and wishes
If a hospital stay is needed
The physician helps decide whether a hospital visit is needed and where to go. If you or your family member is admitted, the physician talks with the hospital team and keeps the family updated.
We have working relationships with the major hospitals in Northern New Jersey and with specialists we refer to often.
Who it helps most
Older adults managing several health conditions at once, people who are homebound or find it hard to leave the house, and people in their first weeks home from the hospital.
Cost
In-home medical visits are private pay.
For some Nexstep aide and nursing clients, physician visits are part of the care plan. We’ll tell you on the first call if that applies to you.
