A physician-led practice for care at home
We do two things and we do them together: in-home medical visits, and private-duty nursing with certified home health aides. Most practices do one. We built both under one roof because the families who need us don’t have a single problem, they have a set of them that keep colliding. A fall leads to a hospital stay, the hospital stay leads to a discharge nobody’s ready for, and three weeks later the aide isn’t working out and nobody’s caught the medication change.
Why I started Nexstep
I trained in emergency medicine, and later spent years building home-based care programs. The same pattern kept showing up: the problems that sent older adults back to the hospital were visible at home first. A missed medication. A rug nobody moved. A discharge plan that looked fine on paper and fell apart by the second week.
Working in population health, entirely with Medicare patients, taught me the same lesson at a larger scale. The older adults who did well at home had someone watching closely and a plan that changed when they did.
Nexstep is set up to do that at home: a nurse who sees each aide client every two weeks, a care plan written by a registered nurse, and a physician reviewing it. If you’d like to talk before deciding anything, call the office.
